The SHAMS HAMZAZAI FOUNDATION (SHF) is dedicated to improving healthcare access and outcomes in our community. To enhance the capabilities of our Blood Bank Health Center, we propose a partnership with Liaquat National Hospital Foundation for the provision of essential blood bank-related consumables, instruments, and equipment. This proposal outlines our SMART objectives—Specific, Measurable, Achievable, Relevant, and Time-bound—along with the project activities, budget, and expected outcomes.

2. Introduction
History of SHAMS HAMZAZAI FOUNDATION
Established in December 2018 in Loralai District, Balochistan, the SHAMS HAMZAZAI FOUNDATION was founded by Shams Hamzazai with a vision to address critical social issues. Over the years, SHF has worked tirelessly in various sectors, focusing on healthcare, education, poverty reduction, and social welfare. We have successfully formed 373 local communities and initiated programs addressing CNIC registration, blood donation, and community conflict resolution through traditional Jirga.
Past Performance and Provided Services
Since its inception, SHF has made significant strides in enhancing community health and well-being:
• Healthcare Initiatives:
o Provided free eye treatment to 1,100 individuals, including 190 surgical operations.
o Assisted over 1,300 patients financially for various health treatments.
o Established the SHAMS HAMZAZAI FOUNDATION BLOOD BANK in 2022, donating approximately 2 million cc of blood, benefitting around 2,400 individuals.
• Community Awareness:
o Conducted awareness programs on COVID-19 and health education, promoting preventive care and hygiene.
• Education Programs:
o Facilitated school admissions for hundreds of students and established 11 community-based Alternative Learning Programs (ALP) in rural areas.
• Sports and Community Engagement:
o Sponsored numerous sports tournaments and provided sports equipment to local schools, fostering youth engagement and teamwork.
With these initiatives, SHF has built a strong reputation as a trusted organization committed to social welfare and health improvement in our community.

3. Problem Statement
Current operations at the SHF Blood Bank face several challenges due to the lack of modern equipment and consumables, including:
• Insufficient blood storage facilities leading to potential spoilage.
• Inadequate testing equipment affecting timely screening of blood donations.
• Limited consumables for blood collection and processing, resulting in operational inefficiencies.
These challenges directly impact our ability to provide safe and reliable blood supplies, compromising patient care in emergencies and scheduled procedures.

4. Project Objectives
Specific
To procure and implement essential blood bank-related instruments and consumables, including blood storage refrigerators, testing equipment, and collection kits.
Measurable
Achieve a 30% increase in blood processing capacity and a 20% reduction in blood spoilage rates within 12 months of project implementation.

Achievable
Collaborate with Liaquat National Hospital Foundation to identify and source the required equipment and consumables within the proposed budget.
Relevant
Align with SHF’s mission to enhance healthcare access and quality in our community, ensuring a reliable blood supply.
Time-bound
Complete the procurement and installation of all necessary equipment and consumables within 6 months of project approval.

5. Project Activities
1. Needs Assessment:
o Conduct a comprehensive evaluation of current equipment and consumables used in the SHF Blood Bank.
o Identify specific instruments and items required to enhance operations.
2. Collaboration with Liaquat National Hospital Foundation:
o Establish communication with Liaquat National Hospital to discuss procurement options and partnerships.
o Develop a list of recommended equipment and consumables based on best practices in blood banking.
3. Procurement:
o Secure funding and resources for the purchase of identified equipment and consumables.
o Engage with suppliers for competitive quotes and timelines.
4. Installation and Training:
o Oversee the installation of new equipment and ensure compliance with safety standards.
o Provide training for staff on the proper use and maintenance of new instruments.
5. Monitoring and Evaluation:
o Implement a tracking system to monitor the usage and effectiveness of new equipment and consumables.
o Conduct regular evaluations to assess improvements in blood processing capacity and quality.
